The first public release.** This was the version shown at XOOPSDEM 2005 in Brussels, Belgium.

Prior to the first release, work had been ongoing since July 22, 2004, when the Formulaire module for XOOPS was forked and new capabilities were added, starting with the ability to link the options in a dropdown list in one form to the values entered in another form, and the ability to control the visibility of entries based on group memberships.

After another month of work, on September 3, 2004, the first website using Formulize was launched, an internal “portal” for Let’s Talk Science. A copy of the code from soon afterwards survives.

Small scale work continued through December 12, 2004, by which time the code base was referred to internally as version 1.3. And then there was a big push to complete the first public release, version 1.5, by early January.

Prior to the Formulize code base, the ideas that inspired Formulize were developed while building a series of internal websites for Actua from 2000 to 2002. These websites featured a rudimentary application development toolkit, which was designed to be used by non-developers to create whatever interactive database systems they needed. On June 1, 2002, the final version of these tools was released internally. The codebase was destined to never be released publicly.

In 2006, Actua replaced that system with a website based on XOOPS and Formulize 2.1. In 2010, that website was replaced with a new one based on Drupal and Formulize 3.12, later upgraded to Formulize 4.06. That website was completely replaced in 2018 with a new web application using solely on Formulize 5.0. It has been upgraded successively since then.
